- Guarantee of: 8 hours per day - OT to apply after 8 hours per day Hours would be 8am-5pm with 1-2 hours of hospital rounding included depending on how many patients are in the hospital at the time. It would look like 8am-3pm in clinic and 3pm-5pm at the hospital for example. NNCCPA- board certification required. We need the candidate to be able to operate on their own, minimal training will be provided so we need them to proficient.

REQUIREMENTS: ____________________________________________________________________________ Active CA license and CA DEA NNCCPA- board certification required.

PRIVILEGING: ____________________________________________________________ PLEASE ENSURE AN UPDATED CV WITH ALL HOSPITAL AFFILIATIONS IS UPLOADED DURING PRESENTATION. Attached is a new privileging form that will set expectations for what is needed once a provider is confirmed. Please consider starting to ensure your provider has all of these items to ensure they can qualify for privileges at the time of confirmation. o All HBS specialties require 2 years of current Hospital clinical activity in the specialty. o Gaps greater than 3 months must be explained. o Include current number of case logs upon presentation for surgical specialties.
