About the Role

The CORE Psychosocial Rehabilitation Specialist (PSR) provides person-centered, recovery-oriented services to adults

enrolled in CORE who are working toward improved functioning, independence, and community participation. PSR is

designed to help individuals build practical life skills, strengthen coping strategies, and improve their ability to function

in the environments where they live, work, learn, and socialize. Services are skill-based, goal-directed, and focused on

helping individuals achieve valued life roles and greater self-sufficiency.

Key Responsibilities

  • Provide Psychosocial Rehabilitation services to adults with behavioral health needs in home and community based settings
  • Support individuals in developing practical skills related to daily living, communication, social interaction,

organization, and community functioning

  • Assist individuals with building routines, managing responsibilities, navigating appointments, and improving

independent living skills

  • Teach and reinforce coping strategies, problem-solving techniques, stress management, and self-management

skills that support recovery goals

  • Help individuals strengthen community integration by developing confidence in using public resources,

transportation, and natural supports

  • Deliver skill-based interventions tied directly to goals identified in the Individual Service Plan (ISP)
  • Coordinate with care managers, clinical providers, and other treatment team members as appropriate
  • Maintain timely, accurate, and Medicaid-compliant documentation for all services provided
  • Participate in supervision, trainings, and interdisciplinary meetings as required

Qualifications

  • High School Diploma or GED required
  • Bachelor’s degree in human services or related field preferred
  • Experience working with adults in behavioral health, rehabilitation, social services, or community support

preferred

  • Strong documentation and communication skills
  • Ability to work independently in community settings
  • Reliable transportation preferred

Job Type: Part-time

Pay: $35.00 - $40.00 per hour

Work Location: In person
