This position partners with providers, facilities, and internal teams to analyze performance data, identify opportunities for improvement, and facilitate implementation of evidence-based quality improvement strategies. The CQIA II serves as a clinical resource and advisor, helping drive measurable improvements in patient care, regulatory compliance, and healthcare quality outcomes</span></h1><h1 style="text-align:center;"><span style="font-size:14px;"><span style="line-height:115%;"><span style="font-family:Aptos, sans-serif;">Working collaboratively with interdisciplinary teams and experience working with dialysis centers, the Clinical Quality Improvement Advisor II - Registered Nurse  assesses complex needs, develops person-centered care plans, and connects individuals to critical resources that improve health outcomes and quality of life. Provide subject matter expertise related to hospitalization, vaccination, patient and family engagement, and mental health/depression screening. This position also supports the organization’s mission through advocacy, education, and outreach across diverse and underserved populations.</span></span><br><br><span style="line-height:115%;"><span style="font-family:Aptos, sans-serif;">The Clinical Quality Improvement Advisor II - Registered Nurse position is a full-time, 40 hours per week, remote position. T</span></span>he successful candidate would have excellent written and verbal communication, organizational skills, and attention to detail. Registered Nurse with ESRD experience required, Nephrology and case review required with a minimum of 7 years clinical. This position requires travel up to 15-60%.
