Client Care Coordinator

Position Overview

The Care Coordinator is responsible for managing the full client lifecycle—from initial consultation through ongoing care management—while supporting caregiver recruitment and ensuring high-quality service delivery. This is a fast-paced, field-based and office-integrated role requiring strong judgment, communication, and follow-through.

This position is expected to run through November, with the potential to transition into a permanent role based on performance and business needs.

Primary ResponsibilitiesClient Intake & Care Planning

  • Conduct in-home consultations with prospective clients and family members
  • Assess physical, cognitive, and psychosocial needs to develop individualized care plans
  • Set clear expectations with families regarding scope of care, scheduling, and pricing
  • Partner with internal team to ensure timely start of care

Caregiver Matching & Onboarding to Case

  • Identify and match appropriate caregivers based on client needs, personality, and logistics
  • Facilitate caregiver/client meet & greets and ensure alignment before start of care
  • Troubleshoot mismatches quickly and decisively

Ongoing Care Coordination

  • Serve as primary point of contact for assigned clients and caregivers
  • Conduct regular follow-up with both clients and caregivers to ensure satisfaction and quality of care
  • Proactively identify and resolve issues related to care delivery, scheduling, or caregiver performance
  • Maintain accurate and timely documentation of all client interactions and updates

Supervision & Quality Assurance

  • Conduct supervisory visits in clients’ homes to assess care quality and caregiver performance
  • Provide coaching and feedback to caregivers in the field
  • Escalate performance concerns and partner with leadership on resolution

Team Communication & Operations

  • Communicate all client needs, schedule changes, and updates clearly to the office team
  • Collaborate closely with scheduling to ensure shifts are staffed appropriately
  • Maintain strong internal alignment to support seamless operations

Caregiver Recruitment & Hiring Support

  • Participate in ongoing caregiver recruitment efforts
  • Conduct phone screens and first-round interviews with caregiver candidates
  • Evaluate candidates for both skill and cultural fit

Job Type: Full-time

Pay: $25.00 - $30.00 per hour

Education:

  • High school or equivalent (Preferred)

Experience:

  • Customer service: 1 year (Preferred)

Ability to Relocate:
